Provides medical assistance and supervision for the elderly and disabled in a warm and friendly environment.
Our
professional staff assesses each individual's needs in collaboration
with family members and the personal physician. A threapeutic
plan of care is developed and reviewed every 90 days with regards to:
- Health Status
- Mental Status
- Nutrition
- Rehabilitation Needs
- Advocacy
- Counseling
- Recreation
- Social Needs
Transportation
to and from the Center is available in Calvert and Southern Ann Arundel
Counties. Arrangements can be made for local medical
appointments.
Family plays a very important role in our
Center. From the time of admission, family members provide input
on care planning and on-going evaluation of our program. Care
plan meetings are held periodically throughout the year. Family
members, caregivers, and other service staff are invited to attend
these meetings.
FHS is open Monday through Friday except for predetermined holidays. Hours of operation are: 7:00am to 3:00pm. |
